In order to claim your CPD hours, after watching the video, please follow the instruction below:

1 – CPD Reporting and Certification Form (by ICCRC)

2 – CPD Agenda provided (by IMLIN)

 

For your convenience, we have attached CPD Agenda & partially filled CPD Reporting and Certification form. You must fill the other parts as instructed below:

  • Part 1 – RCIC’s Information

Please write your first name, last name, and RCIC#

  • Part 3 – Watched an approved CPD event ……….

Please write the date that you have watched the video

At the end of the form, please sign & date the form

You MUST mail three (3) documents for each CPD course; CPD Reporting and Certification Form, Agenda and your proof of payment.

 

Please send them to ICCRC head office. ICCRC’s mailing address listed on its forms and its website.

 

Note: The video will be valid until December 09, 2019. Please watch it before the end of validity date. This video will not be available after the expiry date.

 

NOTE: To apply credits for this CPD course in 2019, you must watch the video by expiry date December 09, 2019.

All reporting forms and supporting documents MUST be received by the ICCRC headquarters, no later than January 31, 2020.

 

Please remember it is your obligation to mail above documents to ICCRC and follow up with them to earn the credits for this course. IMLIN has no responsibilities in this regard.
